My paycheck is incorrect
If you feel there is an error on your paycheck, please fill out the form and add details about why you feel there is an error. Our payroll team will review and get back to you within 1-2 business days.
Where is my dental and vision cards?
If your dental or vision insurance provider is MetLife, you will not receive your card in the mail. If you would like the card information sent to you, fill out the form above and our team will get back to you within 2 business days.
If you do not have MetLife and have not received your insurance card, fill out the form above and our team will submit a request on your behalf to the carrier.
How do I increase my HSA contribution?
If you are currently enrolled in HSA benefit plans, fill out the form above with the requested change amount per pay period. Our Benefits Team will get back to you within 4 business days.

How do I add my dependent's social security number?
Login to your HR account and click on the tab “Main”. On the left side, click on “Account Contacts”. Then find the dependent and click the three dots under “Actions”, select “View/Edit”. Enter the social security number and click “Save”.
If the person is it not listed, Click “+Add”. Fill in the required information in the fields – First Name, Last Name, Relationship, Birth Date (optional), and Social Security Number. Click “Save”.
